Book of Remembrance 2024
We are working on our Book of Remembrance for the High Holy Days, which begin October 2, 2024. Please take a moment to complete this form so that you can list the names of your dearly departed in the Book of Remembrance for 5785/2024. We respectfully request a minimum donation of Double Chai ($36) to include your listing in the Book of Remembrance. The Book of Remembrance will be emailed to all Congregation Shalom members and others who list names prior to Yom Kippur. We will print a limited number of copies for those attending services on site. We would like to include the names of your departed loved ones, so please complete this form NO LATER THAN SEPTEMBER 20, 2024. We must adhere to the September 20 cutoff date in order to have the Book of Remembrance ready by Yom Kippur. Thank you for helping us meet this deadline!
